Your First Visit to Our Brattleboro Dental Practice
We want your first visit to be as pleasant and comfortable as possible. We also think it is easier when you know what to expect.
During the Visit
You will receive a complete dental examination including assessments of your periodontal health, restorative needs, and an oral cancer screen. In order to render an accurate diagnosis, a diagnostic set of digital images (radiographs) is required. Depending on the complexity of your case, we may ask you to return to the office for a separate consultation.
If you are in extreme pain, we will make every effort to relieve your pain through emergency treatment, and then schedule you for the necessary follow-ups.
What to Bring
Please bring the following information to your first appointment:
- A list of medications you are currently taking.
- X-rays if applicable: if a dentist or physician has taken x-rays of your mouth within the last 12 months, please have them forwarded to our office as they may be helpful in your diagnosis. Digital X-rays can be emailed to us at firstname.lastname@example.org. If additional images or films are necessary, they can be taken at our facility.
- If you have dental insurance, bring your current insurance card. If you don't have a card, we will need the subscriber number, group number, and phone number.
- Please print and complete the Patient Forms at home before your first appointment. If you need assistance with downloading or printing the questionnaire, please contact our office. The questionnaire uses the FREE Adobe Acrobat Reader which can be downloaded by clicking the icon below.
Important: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
Please alert the office prior to treatment if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulation therapy, etc.)
